Falls are the leading cause that many older adults lose their independence! The leading cause of injury among the elderly population is falling. Falls affect the quality of life of the elderly. Seniors who fall experiences physical and emotional trauma. Physical trauma includes broken bones, bruises, lacerations, and head injuries are a few of the results of falls.

Many who have had a fall are fearful of falling again. One out of every three elderly falls annually. One out of every fall in the elderly results in a severe injury. The likelihood of repeated falls is very high for a senior. Falls lead to over 750,000 hospitalizations per year. It sometimes takes up to a year to recover from a fall. Many seniors are admitted to a nursing home and never return home.

Falls are preventable. One or more risk factors lead to falls among the elderly population. Poor physical condition, health issues, and the home environment can increase the risk of falls. Arthritis, poor eyesight, taking multiple medications, dehydration, and lack of grab bars in the bathtub are a few examples of risk factors.

Identifying and addressing risk factors prevents falls. The goal of our fall prevention program is to identify and address the risk factors that lead to potential falls. Hospital 2 Home Transition

We offer supervised care transitions provided by registered nurses to prevent unnecessary hospital readmissions. Our care team works with the client, the family and the other members of the healthcare team such as the discharge planners, physicians and the home health nurses, physical, occupational and speech therapist. The goal of our hospital to home transition care is to reduce hospital readmission rates.

Our services include services necessary to assist the client's transition safely out of the hospital to home. These services include

  • Coordinating communication between other members of the healthcare team
  • Follow-ups with families and discharge planners
  • Assistance with discharge orders
  • Coordination of transportation to  physician appointments
  • Prescription pickup
